You will be referred to us or find us on your own. Either way, we will conduct a preliminary phone call to determine initial needs and gather necessary pre-requisite work. Then schedule a intake service in the home or wherever you reside.
Intake Service in the Home
Our caring case managers will visit with the patient, and any caretakers in their home to complete an evaluation to better understand the patient’s situation, health condition and review their health insurance coverage. All of which will provide the patient with the best choices.
Choose Best Care Provider
Once the free in-in home consultation is complete and it is determined services are needed, the patient will be able to choose the desired medical service needed.
We will then complete the necessary paperwork to secure a required signed order from the patient’s physician. If they do not have a primary care physician, we can assist them in finding one that fits their needs.
We will then facilitate the onboarding process by introducing the patient to the specific medical service agency they selected and ensure there is a comfortable transition to the care that will be provided.
From the time we first see the patient, to when the patient leaves our program, we are there every step of the way to ensure they are comfortable and satisfied with the services they receive from all involved.
